Mentorship Intake Form

Please answer the following questions to help us understand your story, creative goals, and how we can best support your project. Copy these questions and your answers into an email and send to kerrabolton@gmail.com. Then we will reach out to you to schedule our intake call and begin our work together.

1. What story or project are you currently working on (or want to begin)?

Briefly describe the personal, creative, or professional work you’d like mentorship on.

Example:

I’m working on a memoir about surviving my mother’s fundamentalist religious cult and how I rebuilt my life as a public school teacher. I’ve kept journals for years and want to turn them into a narrative that’s both honest and empowering. Eventually, I hope to adapt it into a screenplay.

2. What stage is your project in?

Choose the option that best describes your current stage:

  • Idea or early brainstorm

  • Rough draft in progress

  • Full manuscript or detailed outline

  • Revising or adapting an existing work

Example:

I’m between a rough draft and full manuscript. I’ve written about 25,000 words, but the structure isn’t clear and I keep rewriting the beginning.

3. What is your ultimate goal with this story or project?
